When one drive from our ISIS died, we got an error while consolidating. We also got some error logs in the ISIS web view. Login to the ISIS server using VNC or connect a monitor. Then look for the raid controller software ISIS 5000: Start->Programs->MegaRAID Storage Manager->StartupUI Login and the you see all drives in the tab "Physical"

Have you checked the hardware if a drive is dead/dying? Have you try to reboot? We have been suggested to make a reboot every 4 weeks to prevent harddrives to fail.
