Thanks for working through this.Hoping you could help with a few questions:
It was mentioned about the 64-bit aspect of things. Can you confirm you're booted into 64-bit for us?
Is there any information being written to the Avid console when you are seeing this behavior?
Was the move from OS 10.6.x to 10.7.x an upgrade? You mention starting from a clean install, but of which OS?
Lasltly, in the past I've seen bits remain behind when uninstalling a version, then moving to a new one. I wonder if that might be happening here. Can you walk us through your uninstall and move to the new version?

Also note that OS X 10.7.1 or 10.7.2 are supported, not 10.7.0.

I checked in the activity monitor and it said that everything was running 64bit.
Im not sure about the avid console. I will check that tonight and let you know.
The clean install was os x 10.7. Booted from the Lion USB drive and used disk utility to erase the drive and installed from
I used migration assistant to copy over documents and settings, not applications.
I am running 10.7.2.
It may be some freaky thing. I'll try a couple of things like uninstalling The blackmagic software and see if it makes a difference
Ok. Unistalled everything and reinstalled MC6 without Blackmagic Desktop Video 9. It worked just as it should. Installed Blackmagic Desktop Video 9 and the problems started. It will not play and the error keeps showing up in both source and record monitors. Would anyone have any idea why it wouldn't work?

Can we confirm that removing the card eliminates any error?
Removing the BMD software solves the problem but you obviously cant monitor out. I am working with 720p footage. The problem may be there. Annoying because I was able to work in FCP with that format. I've had one response from BM and they said to try a project in 1080i/29.97. I did and that seemed to work. I tried a few other formats and couldn't get any results. I'm waiting for another response from them.
I am seeing the EXACT same errors trying to play video through a KONA 3. Downloaded correc Kona drivers as well as AVID IO plugin from http://www.aja.com/support/avid/index.php
If I uninstal AJA all works well. I am also getting the same console errors as Meps while Kona is installed.

i have the same issue with decklink extreme 2 on a win7 64 bit engl. sp1/HP Z400 with Blackmagic Desktop Video 9 but only sometimes: when scrubbing fast through clips or timeline with linked AVCHD stuff. After several seconds of srubbing the app seems to hang/freeze but waiting 20-30 seconds the error occurs and when i click then into another position of the clip, the video comes back and i can work on...

I was getting the same error using MC6 when trying to access old project edited on MC5, but I figured out a solution.
I am running MC6 on a Dell T5500 with 64bit Win7 and a Matrox MX02 LE Max. My matrox box was set to output 1080i@29.97fps but my project was edited in 720p/23.976. I changed the output to 720p/59.94 and it works fine now.
